Actively Dying
Updated September 2020. A colleague had to fill this form today and was less than completely thrilled with finding out there was yet another Fraser Health form to fill out. I have made it into an eform. It has a link to an image which is the back side of the form, it has a time stamp, and I made the date fill out in the d/m/y format the form requested. The image is horrid, but I could not get an electronic original. JohnR.
