GAD-7 update
From John Yap - I am working on a tool for managing chronic non-cancer pain and one of the tools is the GAD-7. There is a reference to the scoring which I've added to the most recent version of the eForm, nicely updated by Herb Chang. It's zipped up, ready for Oscar Import. Hopefully there are no offending apostrophes in this html.
Please let me know if it does not unzip properly. The HTML file name is the same, with only 1 line modified to add the reference link.
