Measurements eform with BMI calculator that writes to the encounter pane

March 18 - Minor cosmetic changes to instructions etc. --- March 17-2015 - Form will now convert waist in inches to cm, and write both values to the encounter. Comment field is available to designate L or R arm. Other cosmetic changes. March 16 - 1) Created a "BlankAll( )" function and set that as an onload event, as well as for a new "Reset" button. Rationale: I tested the previous version of your form at work today a few times, and I found that the old values pre-fill behaviour of the form can quite easily lead to erroneous data entry, if you submit the form in the heat of the moment with some of the old numbers before updating. So I am worried about this eForm behaviour potentially triggering extra support calls from lay MOA users to the long-suffering OSPs out there. In my mind it would be better if the default behaviour was for the form to load with blank fields. 2) Due to the above I also removed the warning about accidentally submitting the form with old pre-filled values 3) I re-purposed the "reset" button as a "Show old measurements" button, since that is still a useful function. It just shouldn't be the default behaviour. 4) I moved your "clear all fields except height" button below this 5) I commented out the "write to subject" button and instead added the "WriteToSubject( )" function to the other submit buttons. I can't think of a reason not to write the numbers to the subject line by default. 6) Some more feng shui tweaks -----March 15 2015 - This is version 14. Update - now has a button to clear all measurement fields except height, has a comment field beside pulse for rhythm. Previous verison description: It will convert imperial to metric. It will calculate bmi. It will capture BP HR RR and Waist Circ and store them in measurements. --- NB--- Check your measurements to make sure you have: WAIS , BP , HR , RESP . You can choose to 1) write the information to the subject line 2) write the information to the encounter pane 3) submit the form (will write to measurements on submit) 4) write to the encounter pain and submit 5) print and 6) print and submit. It also will write imperial measurements to the encounter pane. This version has the js file imbedded in the eform, so that only 1 file has to be uploaded to the eforms. If you are already in an encounter pane and part way through a note - an information box will come up which says "Your current note has not been saved. Click OK to save it or Cancel to continue editing the current note." - This sounds counter-intuitive, but you should click cancel and it will write to the encounter. JohnR.
