Opioid Manager
Update April 22nd: I took your comments to heart and put the opioid manager on the Bernstein Diet! It is now a svelte 53 KB size, down from 71 KB, using stylesheets extensively. I've resorted to a full background, thanks to the much improved image from JohnR. The new images are slightly smaller, yet much sharper. There is a new pop-up calculator that does the MEQ calculation for you! I always struggled with doing those calculations in my head!
Alas - the backgroundless version - probably only good for Mac users - could only be compressed to 97 KB - down from 146 KB though! Since this one doesn't seem to be system/OS agnostic, I won't post it. Unless someone smarter than me can figure out why the fonts behave differently between Mac and PC displays using the same HTML commands. BTW - wonder how things look on Ubuntu?
Here's the backgrounded version.

Created By John Yap: For those using this e-form, I've updated the e-form to calculate morphine equivalents, and to "link" with the Opioid Monitoring e-form. The old backgrounds should work. I realize that the backgroundless forms generated look fine on a Mac, but display differently on PC's, as the fonts are somewhat different. So, unless someone can get me a high-res version of the background for the PC version, it won't look nearly as nice as the Mac version.
The upgrade is mainly the calculator.
