Uses the same image. Updated to include 1) OB calculator to allow for booking of 3 ultrasound based on patient EDD and weeks gestation required 2)Automatically determines if patient is Diabetic and if patient is on metformin 3) Pulls latest egfr and creatinine from the measurements 4) has a button link to the lab requisition. Please note for lab button to work some editing needs to be done. Pull up the lab form on your server. Look in the address bar at the top for the address for your lab form on your server. You are looking for the portion of the address that reads fid=253 - your 3 or four digit number will be different. Go to the function in the header that is labeled eLink1() (in this new imaging eform) and replace the three digits in fid=253 with the 3 or 4 digits that you found in the address for your lab form. Please note All the stuff that works in this form was made by David and Shelter, and anything that doesn't work is probably my fault. JohnR. ....Further update to add referral physician to the "copy to" section and a check for patient's pregnancy status to check the proper box and add "Maternity" to "copy to" section if pregnant. JohnR -----------May 16th update ---------------- Have hived off the calendar code - see js script ( which will need to be loaded in to images) - this is now a stable eform again. Does all of the above. There are 2 versions - one has "eform magic" that does the following: 1)the form will automatically write in the encounter "Imaging form filled out, message sent to Office Staff" 2)the form will automatically send a message to someone at the front desk - you must edit this where it says <sendto>5000,5001</sendto> with the provider numbers for the people you wish to send to (see in security under admin) If you don't want this simply take the one without the eform magic

- Updated FHA Imaging Form with added locations of hospitals and xray labs in the Fraser Health North regions. Added patient instructions to the pop-up box. Requires the datecalculator.js script to be uploaded to images. No e-form magic. I had to gimp the image slightly to improve the appearance, so you have to upload new image (ImagingFHA092c.png). Take out the section concerning Chilliwack Orthopedic clinic if not relevant to your location. Correct the "fid" to point to your Lab requisition on your own system.
