<p> &nbsp This is a follow up note on ##patient_nameF##.  I am seeing her six weeks after her Cesarean Section.  She reports she is healing well.</p>
<p>ON EXAMINATION: Her incision is well healed</p>
<p>DISCUSSION  At this point in time she is cleared to go back to all her normal activities.  I have discussed future pregnancies with the patient.  She is aware that there should be a 2 year interval between deliveries to allow the uterus to heal.  She is aware that we would offer her the option of VBAC or Cesarean Section in a future pregnancy.  She is aware that she would need to be monitored closely in any future labour, and that she should deliver in a Hospital that has immediate Cesarean capabilities.  She is also aware of the reasons to be assessed in Labour and Delivery including bleeding, ruptured membranes, labour, decreased fetal movement, or incisional pain.</p>
